The Caregiver Coach is an integral member of a patient’s multidisciplinary care team. The role of the Caregiver Coach is to ensure that caregivers successfully integrate into Equip treatment by adhering to their care plans and fully engaging in treatment. This goal is achieved through proactively identifying and addressing barriers to treatment, identifying opportunities to personalize Equip’s programming to address family needs, and building caregiver skills. The Caregiver Coach is accountable for balancing patient engagement in treatment and adherence to the care plan with patient satisfaction and retention in treatment.
